Difficulty Levels - What difference do they make?

I've asked this on the steam forum and read the manual and loads of other forum threads.  I've even tried rummaging through the script files.  But I can't find a clear answer as to what the impact the different difficulty settings have on the way the game plays.

They obviously affect how much starting cash you get, but I think they also affect the AI behaviour and possibly a few other things.

Has anyone ever documented this?

results from a quick search (scripts).
Not sure if it is exhaustive, or even correct (did not verify in details)

things difficulty seems to affect:

- your starting cash/conditions
- ai starting cash/conditions
- how soon AI starts acting (affects initial number of rounds they do nothing, giving the human player a head start)
- AI buys better equipment on higher difficulties
- hospital stocks(?) higher difficulty seems to reduce the maximum stock ai will produce
- missions
- some game formulas (e.g. calculating success chances)
- reduces some rewards (xp and money?) as difficulty increase
